The Pines Home Health and Hospice believes in the power of teamwork. As an RN Hospice, you will have the opportunity to plan, organize, and direct hospice care, emphasizing community health education and experience. You will work closely with the interdisciplinary group to meet the needs of our patients and their families within their homes and communities.
Responsibilities:
- Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
- Provide professional nursing care utilizing the nursing process
- Assess and evaluate patients' status, write and initiate a plan of care, and regularly re-evaluate their needs
- Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions
- Initiate appropriate preventive and rehabilitative nursing procedures
- Administer medications and treatments as prescribed by the physician
- Counsel patients and families in meeting nursing and related needs
- Provide health care instructions and assist patients with activities of daily living
- Act as a Case Manager and coordinate patient care for assigned caseload
- Communicate with physicians and community health-related persons to coordinate the care plan
- Teach patients and families self-care techniques and provide medication, diet, and other instructions
- Maintain accurate and relevant clinical notes regarding patients' condition and care given
- Ensure a safe environment for patients and assist in providing continuity of care
Requirements:
- Graduate of a National League for Nursing accredited school of nursing
- Minimum of two years of experience, with at least one year in public health, home care, or hospice nursing preferred
- Current licensure to practice professional nursing in the state
- Excellent observation, problem-solving, verbal, and written communication skills
- Ability to organize and prioritize workload independently
- Current CPR certification
- Valid driver's license and insured automobile in good working order
- Self-directed and able to work with minimal supervision
