Job Title: Admissions Coordinator – Nursing Home (New York State)

Job Summary: We are seeking a highly organized, compassionate, and detail-oriented Admissions Coordinator to join our team at [Nursing Home Name], a reputable healthcare facility dedicated to providing exceptional care for our residents. The Admissions Coordinator will be responsible for managing the intake process, assisting prospective residents and their families, and ensuring compliance with all relevant state and federal regulations. The ideal candidate will have excellent communication skills, a strong understanding of the admissions process, and a commitment to delivering outstanding customer service.

Key Responsibilities:

  • Admissions Process: Coordinate the entire admissions process, including receiving inquiries, conducting assessments, and managing documentation for new residents.
  • Resident Evaluation: Collaborate with medical professionals to gather medical history, assess eligibility for nursing home services, and ensure all necessary information is obtained.
  • Compliance: Ensure all admissions meet the legal and regulatory requirements set forth by the New York State Department of Health and other governing agencies.
  • Family Communication: Serve as the primary point of contact for prospective residents and their families, providing clear information about the facility, available services, and the admission process.
  • Documentation Management: Maintain accurate and up-to-date resident files, ensuring that all required documentation is collected and properly filed.
  • Insurance and Financial Coordination: Work with insurance providers and families to verify coverage, ensure billing is accurate, and address any financial concerns related to admissions.
  • Resident Orientation: Provide families and residents with orientation materials, including an overview of facility policies, resident rights, and the nursing home’s services.
  • Collaboration: Work closely with interdisciplinary teams including social services, nursing, and financial departments to ensure a seamless transition for residents.
  • Follow-up: Ensure smooth transition into the facility and conduct follow-up with new residents and families to address any issues or concerns.
  • Reporting: Maintain accurate and timely records on all admission-related activities and report on key performance indicators as required.

Qualifications:

  • Education: High School Diploma or equivalent required; Associate’s Degree or higher in Healthcare Administration, Nursing, or related field preferred.
  • Experience: Minimum of 2 years of experience in admissions, healthcare administration, or customer service, preferably in a nursing home or long-term care setting.
  • Skills & Abilities:
    • Knowledge of New York State healthcare regulations, specifically related to nursing home admissions.
    • Strong communication and interpersonal skills, with the ability to interact effectively with residents, families, and staff.
    • Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
    • Attention to detail and strong organizational skills.
    • Ability to handle sensitive information confidentially and professionally.
    • Compassionate and empathetic demeanor with a commitment to quality patient care.
  • Certifications: CPR and First Aid certification preferred, but not required.

Physical Requirements:

  • Ability to lift up to 25 lbs.
  • Ability to sit or stand for extended periods of time.
  • Ability to navigate the facility and interact with families and residents.

Work Environment: This position is primarily based within the nursing home facility and involves regular interaction with staff, residents, and families. The role may involve occasional evening or weekend hours as needed to support admissions.
