Primary Responsibilities:

ESSENTIAL JOB FUNCTIONS:

1) Knowledgeable of Illinois Supportive Living Program Regulations and compliance needs.

2) Assist in ensuring nursing staff are providing care and services in accordance with the Illinois Nurse Practice Act.

3) Lead, mentor, engage teams to maximize their contributions including, recruiting, and assessing, training, coaching, and managing performance.

4) Work alongside of nursing staff when scheduled or needed to perform nursing duties.

5) Assist in the overall direction, coordination, and evaluation of the nursing department. Carries out supervisory responsibilities in accordance with Parker’s policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; performance management; addressing complaints and resolving problems.

6) Assist with duties such as recruiting and training CNA/nursing staff in tandem with DON.

7) Assist with scheduling and assigning tasks in absence of DON.

8) Ensure resident annual assessments and plans associated with annual assessments are completed and in compliance with HFS supportive living regulations.

9) Ensure that all changes in conditions or outside providers are updated on resident’s quarterly assessments and service plans.

10) Work with DOW in tandem to maintain communication with residents managed care organizations.

11) Assist with coaching and embrace continued communication with nursing department.

12) Assist with quality assurance measures in accordance with supportive living regulations, regarding incidents/accidents, falls, infection control measures, etc.

13) Provide communication to in-house and outside physicians groups and outside providers to ensure up to date service planning for residents

14) Provide assistance with family or resident designee concerns in tandem with DON and nursing staff.

15) On-call responsibilities in absence of DON.

16) RAI/ISP/ISS – completion with co-signature of Registered Nurse.

17) Weekly updates (scheduled) with in house physicians’ group, managed care providers, and outside providers.

18) Assistance with DON for monthly quality assurance measures.

19) Assisting with nursing department day to day needs.

20) Other duties as assigned.

Qualifications and Skills Required:

1) Must be a Licensed Practical Nurse or Registered Nurse in the State of Illinois

2) Have experience and knowledge of Assisted Living/Supportive Living Regulations and compliance needs.

3) Two years’ experience as a licensed practical nurse.

4) Must have a thorough knowledge of State regulations and Guidelines governing Supportive Living Facilities.

5) Able to maintain regular attendance.

6) Self-directed, demonstrating initiative and follow-through

7) Must be inspired to serve the needs of residents.

8) Ability to multitask.

9) Excellent interpersonal, organizational and communication skills.

10) Must have the ability to read, write and follow written direction in the English language.

11) Ability to timely and effectively follow up on all necessary requests from residents and families.

12) Maintain and encourage a positive work atmosphere by communicating in a manner that fosters cooperation.

13) Proficiency in computer skills, Microsoft Office (Windows, Outlook, and Excel) with the ability to learn new applications.

14) Ability to handle stressful situations and work well under pressure.

While performing the duties of this job, the employee is regularly required to stand, bend, talk and hear. The employee will occasionally be required to climb stairs. The employee is required to use hands and fingers to touch, handle and feel, and is required to reach with hands and arms. The employee must be able to lift and/or move objects up to 50 pounds. Must be in good general health and demonstrate emotional stability. Specific vision abilities required by this job include close vision, peripheral vision, depth perception, and the ability to adjust focus.

Works in both interior and external areas with exposure to season-related weather conditions. Requires walking, standing, and sitting up to 100% of the time. Job Type: Full-time

Pay: $70,000.00 - $80,000.00 per year
