What are the responsibilities and job description for the Care Coordinator - Registered Nurse position at the position at Puget Sound Kidney Centers?
We are seeking an experienced RN to provide case management to patients and caregivers and educate them on dialysis care and treatment. The Care Coordinator (CC) is responsible for the provision of quality care management for PSKC patients, and for the documentation following established PSKC standards and practices in compliance with the Washington State Nurse Practice Act.
Education and Experience
- Degree from an accredited school of nursing.
- Bachelor’s degree in nursing preferred.
- Must have a current Registered Nurse (RN) license from the WA State Board of Nursing or compact multi-state nurse license.
- Current certification of Cardiopulmonary Resuscitation (CPR) with a Basic Life Support (BLS) with American Heart Association.
- Minimum of two years' experience in Acute and/or Chronic Hemodialysis.
- Prior experience with patient education and case management preferred.

Location/Hours
This position provides support to PSKC's Mountlake Terrace clinic. This is a regular, full-time position (40hours/week). Typical work schedule is Monday-Friday during clinic hours. Flexibility to work longer days or on Saturdays dependent upon patient needs is required. Ability to cover shifts as a staff nurse is required.
Compensation and Benefits
Hourly pay range for this position is $35 - $58.57 per hour. Wage offers are determined based on multiple factors including previous experience in the same or similar role.
