This role requires the ability to function comfortably both independently and in a team environment, as well as the ability to effectively communicate with others at all levels. Exceptional organization, communication and interpersonal skills are essential for this position.
Key Responsibilities:
- Provide support to individuals to gain independence and participate fully in their community.
- Implement person-centered plans and care plans as determined by the team.
- Support inclusion through participation in community-integrated activities, services, and events.
