I. JOB SUMMARY/RESPONSIBILITIES:
- Plans and coordinates functions and activities related to meeting the psychological and social of patients and their families.
- Supports the case manager and social worker with an emphasis on discharge planning. Performs duties in accordance with all regulatory requirements as well as policies and procedures of The Queen’s Medical Center (QMC).
II. TYPICAL PHYSICAL DEMANDS:
- Essential: sitting, finger dexterity, seeing, hearing, speaking, frequent gripping of an object.
- Continuous: static gripping of an object for prolonged periods.
- Frequent: walking, lifting usual weight of 3 pounds; carrying usual weight of 2 pounds; reaching at shoulder level; repetitive arm/hand motions.
- Occasional: standing, stooping/bending, kneeling, climbing stairs, walking on uneven ground, squatting, twisting body, lifting maximum of 10 pounds, reaching above and below shoulder level.
III. TYPICAL WORKING CONDITIONS:
- Not substantially subjected to adverse environmental conditions.
- Subjected to noise and/or vibration due to shared office space, air conditioning and construction work in neighboring department.
- May be exposed to hostile, disruptive, assaultive and inappropriate patient behavior.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
- Bachelor’s degree in social work or related field; or bachelor’s degree in non-related field with two (2) years of experience working in a comparable job demonstrating knowledge of governmental and social agencies and services may be substituted for degree in social work or related field.
- Must meet and maintain credentialing requirements with the State of Hawaii Child & Adolescent Mental Health Division (CAMHD).
- Current BLS certification.
B. EXPERIENCE:
- One (1) year experience in a health related field or social service agency preferred.
