The Financial Case Manager will play a critical role in the recovery process, providing financial assistance and support to clients.
Duties and Responsibilities:
- Maintain open communication and collaboration with the multidisciplinary treatment team.
- Develop individualized aftercare appointments by collaborating with patients.
- Ensure follow-up appointments are made within 7 days of discharge from our facility.
- Coordinate directly with referents and payors to ensure continued care needs are met.
