Essential Job Duties and Responsibilities:
- Assists residents with activities of daily living (ADL) tasks, including but not limited to bathing, dressing, grooming, toileting, transferring, and mobility maintaining resident dignity and respect and in accordance with all policies, procedures, regulations and guidelines.
- Assist with moving and transferring residents into and out of bed, chairs, bathtubs, wheelchairs, lifts, etc. following all safety/lifting and transferring techniques.
- Communicate appropriate information (including incident reports information) and any observed or suspected resident change of condition to a supervisor immediately.
- Assist residents to walk with or without self-help devices as instructed.
- Encourage residents to socialize and participate in scheduled activities and outings, and assist with those activities as needed.
- Respond promptly and courteously to all resident alerts, needs and/or requests for care, and provide assistance with care as needed.
- Observe communicate emotional condition of each resident, promptly reporting all changes to the nurse.
- Follow and promote personal safety precautions and a safe environment for residents by observing infection control practices, following facility guidelines, and reporting unsafe situations to the nurse or management.
- Check each resident per service schedule to ensure that his/her personal care needs are being met in accordance with his/her wishes.
- Ensure resident safety by assisting in the performance of fire or other safety drills.
- Perform other essential job-related duties as assigned.
Knowledge and Critical Skills:
- Be able to make independent decisions and follow instructions.
- Deal tactfully with personnel, prospects, residents, family members, visitors, government agencies/personnel, and the public.
- Capable of working with ill, disabled, elderly, and emotionally upset people within the facility.
- Communicate effectively in a manner that is sufficient for supervisors, team members, prospects, residents, and families.
- Maintain confidentiality in all aspects of the job to ensure resident privacy.
- Knowledge/proficiency of facility EHR.
Education and Experience:
- Must meet all applicable state and federal requirements for this position.
- Prefer 6 months or more of experience in a home care, assisted living, or long-term care facility.
