Department: Resident Care Services
Reports To: Resident Services Director
Objective: Coordinates resident care related to medications by working with all departments, the medical community, families, and administrative staff to provide for resident needs with continuity and an adherence to the scope of practice and licensure for the community. This position provides complete supervision of the medication room, pass techniques, documentation and supervision, and provision of care related to medication in the community. This position is required to have passed the Colorado Department of Public Health and Environment’s Qualified Medication Administration Personnel training and test. This qualification must be kept current. QMAP’s are on duty for their scheduled shifts and may also be personal care providers.

Responsibilities of the QMAP:
1. Resident charts. Keeping documentation current (physician orders, incident reports, etc.).
2. Observe and report resident status changes.
3. Ensure all medication is current and correct, including medication administration forms, physician orders, changes of dose, etc.
4. Ensure the medication room is completely stocked with all required continuous medications, including PRN, over the counter (OTC), and other medication as ordered by the physician.
5. Control medication room access and key assessment.
6. Pour, pass, and assist with administration of medications in accordance with state regulations.
7. Coordinate physician and other medical appointments as necessary.
8. Read all communication notes regarding the community between the caregiver shifts.
9. Gathering residents for activities daily.
10. Adheres to and conveys the community’s philosophy of maintaining elements of self-care and encouraging independence.
11. Receives report from the previous shift.
12. Reads the shift change log/communication log and shower schedule for information and changes. Initials shift change log/communication log.
13. Assists with activities of daily living (ADL’s), environmental orientation, and other care.
14. Escorts residents to and from dining room, and activities while encouraging residents to find their own way.
15. Assists in making and serving meals and dining room duties as instructed.
16. Provides laundry and housekeeping services to residents as assigned.
17. Demonstrates knowledge of infection control via proper handling of equipment, soiled lined, bedside commodes, and adequate hand washing.
18. Documents care given; adhering to facility policies and procedures.
19. Maintains clean, neat, comfortable, and safe environment for residents, staff, and visitors, including housekeeping duties as assigned.
20. Serving and cleaning up during and after meals.
21. Ability to speak, read, write, and understand English.
22. If you are scheduled for a night shift and are assigned as a QMAP, you are required to remain in your house until you are relieved by another staff member.
23. Staff are not allowed to take a break in the med rooms. This is not a break room.
24. You are allowed two fifteen-minute breaks per day and one thirty-minute lunch. Abuse of this policy will result in disciplinary action up to and including termination of employment.

1. Employees are required to stand and walk in sequence, often accomplishing several tasks at once.
2. Employees may cover large distances walking through facility, on feet consistently.
3. Employees are required to have full use of hands and be able to feel, touch, and hear.

5. Required to frequently bend, stoop, squat, and crouch; occasionally required to crawl, climb, kneel, and balance; continuously required to push and pull.
6. Employees are required to lift and carry up to thirty-five (35) pounds and assist in moving residents throughout the facility.

Employees will be Expected To:
1. Follow correct policy and procedures for department and facility Fire/Disaster Plan/Evacuation Plan. Know the location of fire alarms and extinguishers.
2. Follow correct policy and procedure for hand washing.
3. Follow guidelines for proper methods of lifting.
4. All doorways, hallways, and areas are to be left clear.
5. Be familiar with and know the location and purpose of the Material Safety Data Sheets (MSDS).
6. Follow guidelines for department environment and safety measures.
7. Follow guidelines for safe handling, inspection, maintenance, and storage of medications. Report any misplacement or dropping of medications.
8. Follow guidelines for safe handling and storage of chemicals and supplies.
9. Follow correct policy and procedure for reporting of incidents: staff, residents, etc.
