DescriptionThe Peer Support Specialist will be an essential part of the Behavioral Health Team. Provides telephonic and in person support and assistance with connection to outpatient behavioral health, substance use and other community services to patients who have recently been unhoused and/or coming out of treatment. The peer support specialist works on property but may occasionally travel to work with clients. Connects people served with available community services while promoting the use of natural supports and existing community relationships.

FLSA Classification: Non-Exempt, W2

Responsibilities:

For Peer Support Specialists of all Reveille Foundation Programs:

· The Peer Support Specialist will be an essential part of the Behavioral Health Team

· Provides telephonic and in person support and assistance with connection to outpatient behavioral health, substance use and other community services to patients who have recently been unhoused or released from an institution

  • Conducts outreach efforts in the community to find veterans/underserved clients where they are at. Supports outreach efforts 2-3 times each week outreaching to community partners, shelters and street outreach to identify possible clients to enter Reveille Programs
  • Meets with clients weekly and documents at least four progress notes in Warrior Serve and EHR monthly
  • Provides access to nutrition and wellness programs
  • Facilitates client life skills development including recreational activities, budgeting and financial literacy, soft skills, workforce development training and employment

· Caregiver will be expected to provide proof of driver license and auto insurance upon request

· PWS: Peer Wellness Specialist - Adult Mental Health, Adult Addiction, Family Support, Youth Support

· Required upon request: Driving may be necessary as part of this role

QualificationsRequirements:

· Within 30 days of hire: THW Certification in the state of Oregon as a peer in one of the following:

  • QMHA-R – Qualified Mental Health Associate, Professional or other credentials
  • PSS: Peer Support Specialist - Adult Mental Health, Adult Addiction, Family Support, Youth Support
  • PWS: Peer Wellness Specialist - Adult Mental Health, Adult Addiction, Family Support, Youth Support
  • Driving may be necessary as part of this role. Caregivers are required to comply with all the state laws and requirements for driving. Caregiver will be expected to provide proof of driver license and auto insurance upon request.
  • 1 year Providing peer support services

Preferred Qualifications:

  • CADC I (Certified Alcohol and Drug Counselor)
  • Experience working with patients with chronic, acute and contingent crisis

· High school diploma/GED certificate

· Social Security card

· Driver’s License

· Birth Certificate (Certified copy);

· Any other documents, certificates, resumes, performance evaluation reports, etc., which show your abilities and training for the position

3. Additional selection steps - include a background investigation.

· Employment is contingent upon successful completion of comprehensive background investigation
