The Front Desk Coordinator Bilingual Spanish Full Time is the first point of contact for the entire organization, which requires a positive attitude, professional appearance. This position is Monday through Friday 9 am to 5 pm. The Front Desk Coordinator is responsible for reception, answering phones, scheduling new and existing clients, scanning and faxing, filing, communicating with doctors and other healthcare professionals in the community, communicating with our staff, and assisting with administrative duties as assigned by the Program Director. This position performs highly responsible work that will result in a well-organized and efficient front desk area that is oriented to customer service and demonstrates absolute adherence to company policy & procedures. Responsibilities
1. Answers phones, receives all intake information for paraprofessional cases and enter the appropriate information in the computer
2. Completes intake forms and routes them appropriately for admission approval
3. Assists in the scheduling of appropriately skilled person to fill the case, inputs scheduling information on the computer and continues to assist in the coordination of the scheduling
4. Schedules replacement workers as needed and inputs scheduling information
5. Assists in communication with patients/families on home health aide related issues
6. Assists in coordinating services provided to patients
7. Assists in the maintenance of case management notes related to personal problems and/other situations that arise with staff including those relating to patient care
8. Communicates on a routine basis with contract agencies to maintain smooth relations and to obtain needed information
9. Inputs patient visit information to the Patient Management System on a timely basis and review data regularly
10. Inputs and reviews the following clinical information on a timely basis to include, referrals, admissions, readmissions and reinstatements, care plans, discharge and patients not admitted
11. Performs offices procedures, such as sorting and filing records and photocopying
12. Assists in relaying messages to field staff, office staff and community liaisons
13. Performs other duties as assigned by the Supervisor.

Pay: $16.00 - $17.00 per hour
