Job Description :
- Coordinate and provide home care services to clients in the Boynton Beach area
- Conduct psychosocial assessments and provide supportive counseling to clients and their families
- Work with community resources to ensure clients receive the best possible care
- Collaborate with other social service professionals to develop care plans and interventions
- Provide crisis intervention and mental health counseling as needed
