Key Responsibilities:
- Direct Support: Provide personalized support to residents, assisting with daily living tasks, such as bathing, dressing, and grooming.
- Maintenance of Records: Accurately maintain records and reports regarding resident progress, goals, and any notable incidents.
- Treatment Plans: Implement treatment plans by teaching residents new skills and encouraging independence.
- Community Involvement: Transport residents to community events, appointments, and other activities, promoting socialization and integration.
- Communication: Communicate effectively with residents, staff, and management regarding important issues and concerns.
Qualifications:
- H.S. Diploma or GED required.
- Excellent oral and written communication skills.
- AMAP Certification, SCIP R, and CPR/First Aid preferred.
- A Valid Driver's License is required.
