Case Management Specialist - Louisville KY

Location: Louisville, KY Position Type: Case Management Specialist Interviews, accepts, and provides comprehensive, long-term, structured, complex case management services for an assigned caseload of clients participating in an established life management program; understands the uniqueness of the client’s history to determine the most effective program plans; develops comprehensive program plan/goals and evaluates client's progress by conducting mentoring and counseling sessions with client and staff; serves as advocate for client to acquire services that will enable them to cope with their environment functionally; ensures constant compliance with funding requirements. Works with clients in the Veterans Outreach Program. About this opportunity: Key responsibilities: Interviews, accepts, and provides comprehensive, long-term, structured, complex case management services for an assigned caseload of clients participating in an established life management program; understands the uniqueness of the client’s history to determine the most effective program plans.Assists clients in locating/securing affordable housing; works closely with clients to transition into the new housing; conducts home visitations when appropriate; ensures that client's living quarters are properly furnished and maintained.Prepares and maintains case records and logs on all assigned clients; ensures the accuracy and completeness of the same; enters pertinent information into the established Homeless Management Information System (HMIS).What we are looking for in you: Bachelor's degree from an accredited college or university in Social Work, Behavioral Science, or a related field,Three years of progressively responsible experience providing direct case management social services, including accessing clients’ needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources,any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.Physical Requirements and Working Conditions: Work requires the ability to lift and carry and/or push/pull light (less than 25 lbs.) materials, supplies, and equipment. Light mobility: duties are usually performed by a combination of standing, walking, and primarily sitting. Work is usually performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.
