Location: 900 W Adams Street Jacksonville FL 32204 Position Type: Non-Exempt Pay Range: $16.85-$18.95 About this opportunity: Identifies, screens, background checks, tests, and conducts intake for clients seeking transitional housing.   Goes through the intake process explaining the program, resources, rules, and policy and procedures. Enters all intake information in the HMIS system. Assigns participants to case managers. Assists case managers with participants in finding permanent housing.  Answers e-mail, telephone, and in-person questions concerning intake and program details. Key responsibilities:   Initiates and updates client data through the Homeless Management Information System (HMIS) for clients entering the Transitional Housing program by completing assessment tools to begin measuring clients’ progress through program. Serves as liaison to outside agencies in order to refer clients to and from Salvation Army programs; maintains regular contact with referral sources to/from other agencies and resources. Works with outside agencies, landlords, and property managers to help establish housing. Promotes Salvation Army programs in the community by providing education and information as necessary and recruits clients to maintain operations at full capacity. Communicates with individuals, families, agencies, hospitals, detox, jail, and the legal system via phone or email to explain program eligibility and availability. Conducts initial intake interview via in person or telephone call requests.  Determines applicants’ initial eligibility by asking questions related to the established program guidelines; schedules interviews to meet with applicants for internal placement or external referral. Verifies clients’ income for possible shelter as service fee charges. Processes admitted applicants by orienting them to the program services, explaining program attendance requirements, rules and regulations, obtaining necessary signatures on formal agreements, initiating case records, and arranging for dormitory assignments. Maintains bed rosters and schedules entry dates. Administers alcohol and drug tests in accordance with Center policies and procedures to determine clients continued sobriety as a condition of program beneficiary. Conducts an inventory of client possessions paying close attention for articles not allowed by program policies such as weapons, alcohol containing hygienic substances, radios and pornographic materials. Maintains information integrity by actively updating clients information through HMIS for various statistical data as required by the program and funding sources; generates reports regarding the Transitional Housing program intake upon request. Participates in participant review meetings to present questions concerning a potential clients’ suitability for TSA programs.   OTHER RESPONSIBILITIES:   Directs clients to appropriate agencies for their special needs. Assist with client orientation process. Performs other related work as required.   What we are looking for in you:   Two year college or technical school degree in social work or a related field, and Two years progressively responsible experience performing clerical work in a substance abuse environment or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.   WORKING CONDITIONS:   Works is performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.   Work may require driving a vehicle where there may be discomforts associated with heavy traffic or changes in weather.
