What are the responsibilities and job description for the Caseworker position at The Samaritan Inn, Inc.?
Provide comprehensive case management services to all single residents including: intake assessment, goal setting, provide mental health diagnostic impressions as needed, treatment planning, weekly case plan development, monitor progress weekly/monthly, individual money management, advocacy and referrals. Coordinate the overall interdisciplinary plan of care for the resident from intake to exit.

Assist resident in developing support systems to meet resident needs by coordinating available services necessary to maintain self-sufficiency.

Attend weekly meeting with resident on a consistent basis.

Work with agencies and service providers to facilitate case coordination and information sharing. Obtain resident records from service providers.

Provide documentation in a timely manner, which may include residents follow up, outcome evaluation, resident contact sheets and evaluations.

Provide weekly and monthly reports.

Provide crisis intervention when necessary.

Communicate effectively about resident concerns with all Program Staff.

Provide referrals and communicate with other agencies on behalf of disqualified residents.

Attend weekly clinical meetings

Be available to Program Specialist beyond working hours for questions or support.
Job Requirements:
At least two years of case management experience

Problem Solving and reflective listening skills

Empathy and patience towards residents

Basic computer skills

Ability to ascend and descend multiple stairs and/or ramps daily, multiple times a day using feet and legs; hands and arms may be used for balance (i.e., using a handrail)

Minimum of Bachelor’s degree in social work, sociology, or psychology.
Required Education/Experience:
Understanding of homelessness and influencing factors.
