Case Manager
POSITION DESCRIPTION:
The Case Manager will organize, facilitate, and manage the coordination of services for Samaritan Well residents. Services are provided to individuals and families who are experiencing homelessness. This process organizes a spectrum of effective, community-based services and supports in order to help residents achieve housing, employment/financial stability, community engagement and improve independent living skills.
QUALIFICATIONS
Required:
- Bachelor’s degree and/or significant related experience.
- Demonstrate proficiency of Microsoft programs, QuickBooks or similar computer programs.
- Current 1st Aid and CPR certification or within 1 month of hire.
- Excellent oral and written communication skills and professional demeanor.
- Strong organizational skills with attention to detail for accurate documentation.
- Ability to work 37.5 hours per week, during M-F 8:30am-4pm.
- Provide On Call services 4:00p.m. – 8:30 a.m. weekends/holidays (alternating with the Executive Director and other staff).
Preferred:
- Bachelor’s degree in social work, psychology, counseling or social service-oriented field.
- Previous case management experience.
- Previous experience in behavioral health, social services and networking with collateral agencies.
- Knowledge of crisis management strategies and intervention techniques.

DUTIES AND RESPONSIBILITIES
- Responsible for the completion of a comprehensive intake on each resident within timelines.
- Responsible for the development of an individualized service plan (ISP) with each resident, establishing goals of service and an action plan to achieve each within timelines.
- Responsible for the evaluation of the ISP for each resident as directed by timelines.
- Responsible for ensuring thorough and timely case notes are completed for each resident from intake through discharge.
- Responsible for assuring all resident case files meet funding requirements.
- Assist residents to obtain important life documents as needed.
- Provide life coaching and face to face services to residents as needed or link them to community supports (job search training, parenting assistance, budgeting etc.).
- Responsible for submitting weekly case management report to the Executive Director.
- Responsible for providing all services in a dignified and confidential manner.
- Responsible for providing resident services within professional boundaries and ethical working relationships.
- Provide support to the Executive Director as needed to include day-to-day operation of the shelters, monitoring schedules, checklists etc.
- Assist with intern and volunteer training and supervision as needed.
COMMUNITY INVOLVEMENT
- Work collaboratively with community organizations
- Attend Inter Agency Counsel (IAC) and Recovery Oriented System of Care (ROSC).
- Positively promote Samaritan Well programming in the community.
- Work collaboratively with agency fundraising efforts
ORGANIZATIONAL RELATIONSHIP
The Case Manager reports to the Executive Director.
Job Type: Full-time
Pay: $17.00 - $20.00 per hour
Expected hours: 37.5 – 40 per week
