Discharge Planner

Location: Venza Care at The Sanctuary at the Woodlands

Compensation & Benefits:

Shifts:

• Full-Time, Days

Key Responsibilities:

• Coordinate discharge planning for residents in collaboration with the interdisciplinary team
• Communicate discharge plans with residents, families, and external providers
• Arrange for home health, outpatient services, medical equipment, and transportation
• Ensure all necessary documentation and discharge instructions are completed and provided
• Educate residents and families about post-discharge care and follow-up
• Maintain communication with hospitals and community partners to support transitions of care
• Document discharge plans and updates in the electronic health record system
• Participate in care plan meetings and quality assurance programs related to discharge outcomes

Qualifications:

• Bachelor's in Social Work, Nursing, or a related healthcare field preferred
• LCSW license Required
• Prior experience in discharge planning, case management, or care coordination (LTC or hospital setting preferred)
• Knowledge of community resources, insurance processes, and discharge regulations
• Excellent communication, organizational, and problem-solving skills
• Ability to multitask and work effectively as part of a team
• Familiarity with electronic health records (PointClickCare preferred)
