The Sharon at SouthPark is a leading senior living life plan community located on a 28-acre campus near the heart of SouthPark. We are a non-profit long term care facility that offers independent living, assisted living, and skilled nursing care. At The Sharon at SouthPark our team member's number one responsibility is to deliver compassionate resident care every time! The Sharon at SouthPark, is seeking a seasoned Director of Nursing for our Skilled Nursing and Assisted Living Department. Founded in 1969, our not-for-profit community is currently home to over 400 seniors and approximately 340 team members. The Sharon at SouthPark is within a five-minute walk to numerous upscale retail establishments, office buildings, healthcare facilities and residential offerings. We recently completed an exciting multi-phase campus master plan that includes a new 42 unit independent living apartment building as well as a fully renovated and expanded health center, culinary kitchen, dining room and amenity spaces. Phase II of the campus master plan is in the initial planning stages and is expected to add a Center for Vibrant Living, and approximately 64 additional independent living apartments. Reporting to the Nursing Home Administrator, the Director of Nursing is responsible for developing , planning, organizing, directing, implementing, and evaluating the 24-hour a day, seven days a week management and overall operation of the Nursing Services Department, in accordance with current federal, state, and local standards, guidelines, and regulations, within the licensed areas of The Sharon at SouthPark. Ensures quality care and adherence of clinical programs, compliance, polices, procedures, managing and controlling budget as well as providing leadership and oversight of the nursing services departments clinical staff. QUALIFICATIONSSuperior communication skills including writing, presenting and interpersonal communication.Demonstrated ability to increase efficiency and continuously improve methods, approaches, and departmental effectiveness. Commitment to continuous learning.Demonstrated ability to see the big picture and provide useful and strategic advice, planning and implementation across the nursing department and to the senior leadership team.Ability to work independently with a high level of confidentiality and discretion.Ability to work with considerable independence, developing processes to accomplish objectives in alignment with broader business objectives.Excellent customer service skills and professional demeanor at all times to interface effectively with all internal and external customers. SUMMARY Responsible for developing, planning, organizing, directing, implementing, and evaluating the 24-hour a day, seven days a week management and overall operation of the Nursing Services Department, in accordance with current federal, state, and local standards, guidelines, and regulations, within the licensed areas of The Sharon at SouthPark. Ensures quality care and adherence of clinical programs, compliance, policies, procedures, managing and controlling budget as well as providing leadership and oversight of the nursing services department’s clinical staff. ESSENTIAL DUTIES AND RESPONSIBLITIES include the following (other duties may be assigned) : Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Provides leadership to the nursing staff to ensure that care standards are met and the highest degree of quality resident care is provided at all times in line with The Sharon at SouthPark Mission Statement, Vision Statement, and Unifying Principles.Provides clinical care through the direct application of the nursing process; perform and document resident assessments, complete Individualized Service Plans, provide or delegate hands-on clinical care as indicated by the plan of care and evaluate resident outcomes.Supervises all activities of licensed and certified nursing personnel in the delivery of safe and therapeutic care. This includes, but is not limited to, managing the clinical operation of nursing care, delegating work assignments for nursing staff, supervising nursing staff on daily job duties, equipment evaluation, and report preparation.Interviews, hires, trains and coaches key nurse supervisors and nurse positions in accordance with established company policies and procedures to promote successful onboarding.Develops, and revises as needed, written job descriptions for all licensed and non-licensed nursing personnel.Supervises the coordination and direction of the training and reviews (to include 30-60-90 day and annual) of licensed and certified nursing personnel to ensure compliance.Supervises, coordinates and conducts annual employee evaluations for all nursing employees in accordance with administrative policies and procedures.Establishes and implements nursing policies and procedures and is responsible for ensuring nursing staff is compliant with all departmental procedures and policies. Stays aware of current federal, state, and local regulations to ensure that all policies and procedures implemented are in accordance with all applicable federal, state and local regulations.Actively identifies department issues, recommends corrective actions and conducts appropriate follow through.Readily and effectively reviews and approves admissions for the Medicare certified beds, Health Center and Assisted Living admissions to maintain or exceed annual, budgeted census. Supervises the ordering of medical supplies and equipment, and the maintenance of adequate inventory, completed by the Central Supply Clerk, to ensure established budgetary measures are met. Verifies all related invoices as appropriate. Performs nurse management rounds in all licensed areas, when present in the building, to observe residents and ensure nursing needs are being met. Provides, or delegates, hands-on clinical nursing care during nurse management rounds. Effectively collaborates with the Medical Director, and other external physicians, pharmacists, and other clinical providers to coordinate clinical and medical care and services for the residents to identify and resolve issues as they arise to ensure the delivery of a high quality of clinical services. .Effectively and openly supervises nurse supervisors and oversees nurses to ensure proper notification of responsible parties concerning pertinent details of residents' conditions occurs.Oversees appropriate, up to date resident assessment, planning and implementation of residents' nursing and health care plans is completed.Is responsible for directing and ensuring that all pharmacy recommendations are followed up in a timely manner upon receipt. Directs, plans, implements and monitors any nursing corrective actions needed as a result of surveys by local, state and federal regulatory bodies in all areas with the licensed areas of The Sharon at SouthPark.Functions as both a team member, team leader, and supervisor to ensure that work is accomplished per protocol and at the highest standards of resident care and quality.Maintains a professional, proactive, and collaborative working relationship with residents, family members, Directors, Leadership, direct reports, team members in other departments, and representatives of external partners of The Sharon at SouthPark groups.Promotes teamwork, mutual respect, and effective communication during daily interactions within the nursing services department and with other departments to ensure that nursing services can be adequately maintained to meet the needs of the residents.Addresses and follows through with resident and family complaints / concerns in a timely manner to ensure patient satisfaction. Reports any complaints to the Administrator.Effectively communicates directly with residents, family members, and members of the interdisciplinary team to include contract medical staff, Directors, and Leadership, to coordinate care and services and respond to and resolve complaints and concerns in a professional manner.Readily complies with company policies and procedures and all related local, state and federal regulations along with accepted professional standards of practice.Actively participates in the daily clinical meeting to discuss census changes, resident changes in status such as falls / wounds, and concerns, and report the changing needs of the resident and to assist in determining nursing needs. Directs and implements next steps from a nursing perspective as appropriate and recommends next level of appropriate resident placement when needed. Leads the clinical meeting in the absence of the Administrator.Directs and develops a written plan of care (preliminary and comprehensive) that identifies the problems or needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care. Ensures that all nursing team members involved in providing care to the resident are aware of the resident’s care plan.Supports and oversees the Antibiotic Stewardship Program lead by the Infection Preventionist and oversees trending / tracking of outcomes. Acts as the Interim Infection Preventionist when needed. Assists the Infection Preventionist Nurse in identifying, evaluating, and classifying routine and job-related functions to ensure that tasks in which potential exposure to blood or body fluids are properly identified and recorded. Monitors and evaluates the infection control program in accordance to current infection control guidelines to prevent the development and transmission of disease and infection.Participates in maintaining the Payroll Based Journaling nursing data accurately and timely and ensure timely departmental entry occurs for timely submission to participating government agencies. Participates in the nursing services department’s planning and preparation of annual budget development. Submits requested budget recommendations and financials to the Administrator for review, recommendations, and / or approval.Directs and oversees the medical supply ordering, inventory, and billing within the medical supply billing software to ensure resident care needs are met and monthly charge capture numbers are entered and recorded timely. Supervises and evaluates the training and compliance of nursing staff charge capture of medical supplies being scanned out to residents accurately.Assists in the preparation of inspection surveys, including staff in-services on survey processes, and participation in mock surveys as indicated.Participates in the facility’s plan of correction response to an inspection survey and implements any follow-up required for nursing allegations.Oversees resident incident reports and concerns daily to identify any unusual occurrences and reports them promptly to the Administrator and / or state agency for appropriate action as necessary.Monitors for allegations of potential abuse or neglect, or misappropriation of resident property and participates in the investigative process.Interprets and communicates policies and procedures to nursing services department staff, and monitors staff practices and implementation.Monitors and directs nursing staffing and scheduling according to operational and budgetary guidelines. Ensures budgeted Hours Per Patient Day are within operational guidelines.Oversees and directs the evaluation of job performance of all nursing services department personnel and implements disciplinary action, as necessary, according to operational policies.Adheres to the defined policy of the Bill of Rights for Nursing Home Residents.Remains current on nursing techniques and advances, as well as latest trends in long term care by attending in-services, conferences, and continuing education programs.Maintains a safe working environment for employees and residents. Supervises the monitoring of nursing employees to ensure that they are following established safety regulations in the use of equipment and supplies. Addresses any issues identified immediately.Effectively researches information and analyzes data to arrive at valid conclusions, recommendations and plans of action to support positive outcomes in the operations of the nursing department.Oversees and supervises the clinical staff development training and education program delivered by the Staff Development nurse, to nurses, medication aides, certified nursing assistants and other departments who provide resident care, or services, within the licensed areas. Supports and performs skill evaluation, a needed, to develop and promote clinical capabilities of nursing department team members.Actively participates as a member of the Quality Assurance and Performance Improvement committee. Leads clinical quality and process improvement initiatives within the community to mitigate risk and improve resident care outcomes. Track, trend and report clinical quality data to identify risk. Reports tracking outcomes of assigned responsibilities at quarterly QA / QAPI meetings with performance measures for improvement as appropriate. Responsible for being on-call to handle any urgent nursing issues or concerns outside of regular working hours.Maintains confidentiality within the department in relation, but not limited to : disciplinary actions; family concerns; staff concerns; staff changes; resident status changes.Develops and maintains a nurse On Call rotation to provide adequate nurse coverage for all shifts 24 hours a day, 7 days a week to meet resident needs and staff to resident ratio.Ensures Daily Staffing Sheets are kept up to date and stored per federal guidelines.Collaborates with the VP of Health Services regarding any new policies / practices prior to implementation.Assists in emergencies and emergency preparedness situations when they arise.Acts in an administrative capacity in the absence of the Administrator. RESIDENT TRANSFERS : In order to ensure the safety of both residents and team members, nursing staff is required to use a gait belt for all resident transfers. Gait belts must also be used when a resident requires limited to moderate assistance for ambulation. Each nurses’ station will have a house supply of gait belts. All nursing team members are responsible to pick up a gait belt at the beginning of each shift and to return the gait belt at the end of each shift. Additionally, The Sharon is a no lift facility. Safe transfers will be ensured for residents and employees by incorporating the use of Sit to Stand or Passive Floor Mechanical lifts. Use of lifts is required if the resident is unable to support at least half of his / her body weight for stand-pivot transfers.
