Position Overview:
As the Community Support Team - Associate Professional, you will play a vital role in enhancing the team-building process within our organization. You will work closely with the Team Lead to coordinate activities, provide psychoeducation, assist with crisis interventions, and contribute to both behavioral and substance use disorder treatment interventions. You will also support clients by connecting them to essential resources and assisting in their recovery and stability journey. This role is instrumental in ensuring that each client receives high-quality support and services in accordance with their Person-Centered Plan (PCP).Key Responsibilities:
- Provide Psychoeducation: Deliver psychoeducation as outlined in the PCP to empower and inform clients.
- Assist with Crisis Interventions: Support the Team Lead and other members in responding to client crises.
- Behavioral and Substance Use Treatment: Participate in interventions targeting substance use and behavioral health needs.
- Relapse Prevention: Help in the development and implementation of relapse prevention and disease management strategies.
- PCP Development & Implementation: Assist with creating and updating the PCP to reflect ongoing progress and changing needs.
- Client Progress Communication: Regularly report on client progress and the effectiveness of current interventions to the Team Lead.
- Intensive Case Management: Provide intensive case management, connecting clients to formal and informal support systems.
- Housing Assistance: Help clients search for housing, engage landlords, write reasonable accommodation letters, and prevent housing crises.
- Resource Connection: Link clients to financial and in-kind resources needed to set up and maintain their households.
- Crisis Mitigation: Act as a point of contact for landlords and support clients facing housing crises, including eviction risk.
- Daily Living Skills Development: Aid clients in building daily living skills to stabilize and maintain independent housing.
- Functional Needs Assessment: Complete assessments to determine the scope of services needed and anticipated outcomes.
- Rehousing Support: Assist clients with finding new housing if needed due to eviction or inability to stay in current housing.
Qualifications under 10A NCAC 27G .0104:
The Associate Professional (AP) in the mental health, developmental disabilities, and substance abuse services (mh/dd/sas) system of care must meet one of the following criteria:
- Graduate of a college or university with a master’s degree in a human service field and has less than one year of full-time, post-graduate experience with the population served. Supervision will be provided by a qualified professional until one year of experience is met. An individualized supervision plan will be developed upon hiring and reviewed annually.
- Graduate of a college or university with a bachelor’s degree in a human service field and has less than two years of full-time, post-bachelor’s degree experience with the population served. Supervision will be provided by a qualified professional until two years of experience is met. An individualized supervision plan will be developed upon hiring and reviewed annually.
- Graduate of a college or university with a bachelor’s degree in a field other than human services and has less than four years of full-time, post-bachelor’s degree experience with the population served. Supervision will be provided by a qualified professional until four years of experience is met. An individualized supervision plan will be developed upon hiring and reviewed annually.
- Registered nurse licensed to practice in North Carolina with less than four years of full-time accumulated experience in mh/dd/sas with the population served. Supervision will be provided by a qualified professional until four years of experience is met. An individualized supervision plan will be developed upon hiring and reviewed annually.
Additional Qualifications:
- Must have reliable transportation and a valid driver’s license.
- Must pass a background check.
- Must have strong organizational, communication, and problem-solving skills.
- Experience in the mental health field or a related area is preferred.
