Job Overview:
We are seeking a dedicated and passionate Qualified Professional (QP) to join our Community Support Team. In this role, you will provide essential services to individuals in need, including crisis intervention, behavioral health support, and ongoing care coordination. You must be willing to be on-call for crisis phone support, travel to client locations when needed, and be flexible with your schedule to respond to emergencies. Key Responsibilities:
- On-Call Crisis Support: Be available for crisis phone calls and emergency client needs.
- Person-Centered Plans (PCP): Develop, implement, and revise individualized plans to help clients reach their goals.
- Client Interaction: See clients at least once a week and be ready to meet with them during emergencies.
- Behavioral & Substance Use Interventions: Assist with strategies to support clients through behavioral health and substance use challenges.
- Coordination of Care: Ensure clients are connected to appropriate services, including psychiatric evaluations and community resources.
- Relapse Prevention & Crisis Management: Provide support and guidance for clients struggling with relapse or crises.

Compensation:
- Rate of Pay: $20-25 per billable hour
- Administrative Time: 6 hours per week at state minimum wage
- Mileage Allowance: $60 per week
Qualifications:
- Licensed Professional: A licensed or certified individual with at least four years of experience in mental health or substance use disorders.
- Master's Degree in Human Services: One year of experience in a relevant field.
- Bachelor’s Degree in Human Services: Two years of experience working with clients in mental health or substance use.
- Bachelor’s in a Non-Human Services Field: Four years of experience in the mental health or substance use field.
