What are the responsibilities and job description for the Group Facilitator position at The Source?
The Source is a Joint Commission accredited addiction treatment facility that offers multiple levels of care to help men and women overcome alcohol and drug addiction. Our mission is to provide a family environment where clients get connected to their mind body and spirit, support groups, 12-step programs, and society.
The Group Facilitator will conduct and or facilitate all scheduled group activities. You will be responsible for progress note documentation which is placed in the EMR.
EXPERIENCE REQUIREMENTS:
Minimum of two years’ experience in the substance abuse field and a minimum of one year of experience in a partial hospitalization program or another mental health setting. Must have a CAC, CAP or Master level equivalent from an accredited organization. Must be able to pass a Level II background check and pass a pre-employment drug screen.
RESPONSIBLITIES:
- Works in a safety-conscious manner that ensure safe work practices are used in order not to pose a risk to self or in the workplace.
- Document the patient’s chart on a regular basis and complete the patient caseload in a timely manner.
- Share pertinent information with the treatment team as obtained from the client, client’s family members, or significant others
- Document provided services in progress notes in a clear, concise, timely manner.
- Complies with company policies and procedures and local, state, and federal regulations.
- Participate as a team member in all treatment team meetings and provide input regarding patients’ needs and requirements.
- Meets with supervisor on a regular basis to assist in identifying and correcting problem areas and or improving services
- Assist in developing aspects of treatment plans and treatment plan updates in collaboration with other treatment team members.
- Share pertinent information with the team as obtained from patients or family throughout the program.
- Effective time management.
- Attend monthly staffing meetings.
- Continue with educational courses and in-services training, per policies and procedures.
- Update knowledge of community resources on a continuous basis.
- Respond positively to changing situations in the work setting.
- Work as a team member, demonstrating maturity under stress and objectivity.
- Participate in obtaining the patient’s informed consent for procedures, treatments, and research, as appropriate.
- Act decisively and effectively in managing staff, patients, and other employees during an emergency.
- Demonstrate good communication skills.
- Report abuse of patients’ rights immediately.
- Perform in a professional manner and role model a positive image of mental health.
- Perform other tasks as needed.
SKILLS/ABILITIES:
- Ability to work as a team member and have management, communication, organizational and interpersonal skills.
- Ability to work under stressful conditions and be flexible in relation to department needs.
- Knowledge of Joint Commission Standards.
- Demonstrates Proficiency in Verbal & Written Communication Skills.
- Skilled in the use of computers and with specific ability to efficiently use Microsoft Office software,
