Job SummaryThis job is for the care of Kaiser Permanente (KP) members in the Skilled Nursing Facility (SNF) setting. The Nurse Practitioner for the SNF rounding program will manage the care of the member, collaborate with other KP colleagues on the SNF rounding team, hospital, ACC / CDU, and interact with the SNF facility staff (including leadership, therapists, social workers, nurses, and pharmacists) The NP will manage the members along with the SNF Provider and Acute Transitional Case Manager. They will escalate facility issues and patient care problems to the appropriate facility and KP staff in a timely manner. We maintain a drug and nicotine free workplace and perform pre-employment substance abuse testing.Major Responsibilities / Essential FunctionsRound on Kaiser Permanente (KP) members at assigned contracted skilled nursing facilities (SNF) daily to address medical needs and concerns, as well as help with care planning going forward. Willing and able to travel between multiple locations to see patients if necessary.Performs medical assessments and physical examinations and identifies and documents problems / issues in the electronic medical record, issuing appropriate diagnostic and therapeutic orders.Collaborates with physician(s) and other interdisciplinary team members to manage medical and social needs of patients, including participation in interdisciplinary team meetings.Performs diagnostic, therapeutic, and nursing interventions; evaluates effectiveness of treatment plan. Initiates and manages certain medication therapy and writes orders per established protocols and physician orders.Discuss goals of care, transition planning, and advanced directives / POLST, in collaboration with the SNF facility staff and the KP Acute Transitions Case Manager.Effectively communicates with other health care providers, including the patient's primary care physician and specialists, to ensure operational efficiency, consistency, and continuity of care.Serves on committees as appropriate; Attends meetings with SNF staff, NP and KP Acute Transitions Case Manager to assist with discussions of ongoing care planning, discharge planning and coordination of care for members.Acts as a resource to program staff and participates in defining nursing standards and practices.Timely documentation of progress notes, discharge summaries, medicine reconciliation, etc. in KP HealthConnect.Participate in Departmental meetings.Obtain and maintain privileges at all assigned skilled nursing facilities. Participates in compliance training and compliant with KP Principles of Responsibility, including awareness on how to report compliance issues and concerns.Enhances professional growth and self-development through participation in health education classes, quality initiatives, other professional activities, educational programs, current literature, in-service programs, and workshops.Demonstrates superior interpersonal skills and cultural sensitivity. Responds to patients and family with complex medical and social needs in a timely, compassionate, and coordinated manner.Demonstrates knowledge and complies with Center of Medicare / Medicaid (CMS) and other regulatory standards.Take night call rotation during the week and throughout the weekend. Ability to work weekends if necessary.Must be able to drive distances between facilities to make rounds, lift 10 lbs. or more, alternatively sit and stand throughout the day, work on-site in a SNF.Minimum Education and Experience RequirementsUnrestricted current APRN license in the State of GeorgiaMaster of Science in NursingQualified for NPI designation and Medicaid Provider IDCertified Nurse Practitioner certificate in adult medicine, family practice, palliative care, or geriatric nursing2 years of working in a Skilled Nursing facility or another practice area with older adultsThree (3) years of clinical nursing in an acute care settingWorking knowledge of all federal, state, local and regulatory requirementsFunctional knowledge of computers and EMR documentation in multiple systems
