JOB SUMMARY: The Clinical Pharmacist is a licensed professional responsible for all facets of the medication order verification, dispensing and documentation process for conventional chemotherapy and supportive care medications provided for the Infusion Center. The clinical pharmacist will be actively involved in the provision of pharmaceutical care including providing drug information to the healthcare team and patients. The clinical pharmacist will comply with company policy and procedures and quality assurance verification guidelines. Responsible for daily supervision of staff assigned to medication preparation, dispensing, and operation of the Infusion Center in accordance with all applicable rules and regulations.
DEPARTMENT SUPERVISION: All pharmacy technician staff.
Duties and Responsibilities:
- Responsible for verification of medication orders including accuracy of dosages and calculations. Responsible for directly overseeing the preparation and dispensing of all medications for the Infusion Center, including supervision of certified pharmacy technicians during preparation of IV admixtures and oral medication dosages, ensuring compliance with all steps of the double check verification process prior to releasing dosages from the Infusion Center.
- Responsible for building and maintaining all chemotherapy regimens within the EMR system.
- Oversee drug inventories including documentation of current inventory, projection of expected usage in concert with ordering, receiving, storage and dispensation tracking of conventional medications.
- Adheres to appropriate safety and quality assurance practices. Assures presence of authorization to dispense.
- Uses a systematic procedure to communicate pertinent patient information to and from other interdisciplinary teams. Provides concise, applicable, and timely response to requests for drug information from healthcare providers. Formulates responses to drug information requests based on analysis of the literature.
- Contribute to the effective operation of the pharmacy department which requires daily accurate documentation. Responsible for facilitating proper communication by demonstrating a professional, positive and helpful attitude in interactions with co-workers both internal and external to the Infusion Center.
- Maintains confidentiality of patient and proprietary information. Observes legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary information.
- In all activities, complies with accreditation, legal, regulatory, and safety requirements. Complies with state/federal laws and USP standards, including Drug Enforcement Administration (DEA) controlled substance regulations set forth by the state and DEA. Complies with policies regarding the perpetual inventory systems for schedule II narcotics, controlled substances and other medications subject to diversion.
- Maintain continuing education, skill assessment testing, competency requirements, certification and licensure for current job title and position. Attends educational seminars or participates in other CEU opportunities in order to promote professional competency and complies with state mandated CEU requirements. Requirements include providing a true and correct copy of current pharmacy license and receipts from required continuing education (ACPE accredited courses).

Qualifications
Basic Education and/or Experience Requirements:
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license as granted by the State Board of Pharmacy.
- One of the following:
- At least one 1 year of experience in sterile IV admixture procedures.
- knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM).
- completion of an ASHP accredited Pharmacy Residency Program.
- completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
- equivalent experience as approved by the Director of Pharmacy and Director of Clinical Services.
Basic Qualifications:
- Basic level skill in Microsoft Word, Microsoft Excel, and Microsoft PowerPoint.
Preferred Qualifications & Interests (PQIs):
- At least 1 year of experience in indirect management of team members, including assisting in the development, training, and assignment of work/projects to other team members.
- Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
