What are the responsibilities and job description for the Social Worker position at The Stayton?
We are seeking an Social Worker to join our community committed to delivering outstanding care to our residents. As a Social Worker, you will play an important role in all phases of the nursing service that meets or exceeds Buckner quality, service, and hospitality standards. What you'll do:
- Work with residents demonstrating behavior or psycho / social problems; mediate disagreements between residents; document acute behavior as referred by staff.
- Develop social care plans to meet state requirements as referred by MDS Coordinator.
- Act as liaison between residents and families and other staff / administration; advocate for residents' rights; handle residents' complaints.
- Assist families / residents in coping with admission and facilitate smooth transition to environment as referred by staff.
- Meet individually with residents or family members to assist with daily task (ensuring bills are paid, maintenance requests, insurance claims forms, etc.) and check on their well being as referred by staff.
- Uphold rules of confidentiality of resident's medical records and other personal data.
- Assist Admissions Coordinator with phone inquiries, tours and admissions.
- Assist in discharge planning with staff, residents, families and community resources as needed.
What you'll bring:
- Requires in-depth understanding of a comprehensive field of knowledge normally associated with the attainment of a Bachelor's degree in a related field. Master's Degree in Social Work required. Minimum 1 year prior related experience in a health care setting required.
- Licensed Social Worker required by community and/or state regulations.
- Requires knowledge of basic health care practices and procedures; familiarity with counseling and education techniques in health care field.
- Requires proficient ability to speak, read and write English.
- Requires ability to use up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently and/or 10 pounds continuously to move objects and/or people.
- Requires ability to walk, stand and sit, sometimes for prolonged periods of time.
- Requires ability to grasp, pull, push, carry or otherwise manipulate objects; ability to perform tasks requiring action of muscles or groups of muscles and foot and/or hand-eye coordination.
