ESSENTIAL DUTIES & RESPONSIBILITIES :
Ensures that the individuals supported are provided person centered programming and are encouraged and supported in making their own decisions to the greatest extent possible.
Ensures that individuals are offered opportunities to participate in their community to the greatest extent possible and in the most integrated setting appropriate to their needs.
Oversees daily program activities, and ensures that the provision of skills training inclusive of community recreation and leisure, money management, personal responsibility, personal care needs, self-regulation, and social interactions occurs daily in accordance with the individuals' life plans.
Follows through on any individual behavior management plans and works cooperatively with behavior support personnel.
Assists in hiring, training and supervising staff in the implementation of best practice models to meet identified habilitative goals. Assists in training and supervising staff in accordance with the DSP workforce transformation competencies and agency requirements.
Works cooperatively with the individuals and the individuals' support teams to identify areas of need and individuals' valued outcomes.
Completes and maintains accurate documentation relating to the various services provided to individuals and their progress toward achieving their identified valued outcomes.
Conducts / attends weekly staff meetings and in-service training programs.
Communicates with parents, group home staff / supervisors, and legal guardians during program hours.
Adheres to The Summit Center Core Agency Values of Adaptability, Accountability, Collaboration, Integrity, Optimism, and Respect. Adheres to The Summit Center employment policies as outlined in the Employee Handbook including but not limited to dress code, attendance and punctuality, and paid time off.
Maintains confidentiality of individuals' information in accordance with the requirements of NYS Mental Hygiene Law and the agency's HIPAA Privacy Policies and Procedures.
Safely transports individuals to / from various community sites, activities, and home.
Maintains financial accounts of individuals, tracking receipts, overseeing purchases, and safeguarding the funds.
QUALIFICATIONS :
EDUCATION and / or EXPERIENCE - High School Diploma required, Bachelor's preferred AND one years' supervisory experience working in the field of developmental disabilities. One year supervisor experience preferred.
CERTIFICATES, LICENSES, REGISTRATIONS, Etc. - A valid NYS Driver's License in good standing and agency approval of driving record according to agency's Policy and Procedures, AND clearance through NYS Central Register for Child Abuse and Maltreatment AND clearance through Buffalo Police Department for convictions of criminal offenses. Must be cleared through NYS OPWDD criminal background check unit prior to first day of employment. Strategies for Crisis Intervention and Prevention - Revised (SCIP-R), First Aide, CPR and other applicable state mandated training may be required. (Agency will provide training). Must be cleared through the NYS OPWDD criminal background check unit and undergo Staff Exclusion List (SEL) and Mental Hygiene Law (MHL) checks prior to first day of employment.
This is a full time position, Monday - Friday, 8am - 4pm.
