Summary
Under the direction of the Human Services Director. The Social Services Case Manager will provide culturally appropriate referrals, advocacy and direct services. This position will coordinate with existing tribal programs and outside agencies to provide comprehensive services to tribal members and their families.
Your Education / Experience
- High School Diploma
- Minimum of two years’ experience working in a related field.
Your Responsibilities
- Serves as the technical expert in social services for the Suquamish Tribe. Requires travel and transporting of clients.
- Advocates for the provision of a comprehensive program of professional social services to the Suquamish Tribe. Works closely with other tribal programs and outside agencies in order to eliminate gaps in the service to clients as well as to increase the benefits for the clients.
- Project approachability and develop trust of the Suquamish community, Kitsap County service providers as well as state service providers. Work together with families to mutually identify goals towards improving the health and functioning of their families.
- Where appropriate provide: Tribal and community referrals; advocacy and aid families in learning self-advocacy skills; case management for families receiving services.
- Maintain client files according to program and professional standards, and regular monitoring and assessment of these files.
- Produce and distribute reports as appropriate to ensure optimum continuity of care. Submit a monthly report of program activities to Human Service Director.
- Attend and participate in various family, staff, and community meetings whose goals directly or indirectly relate to the Social Services programs’ goals and objectives.
- Maintain the highest standard of ethical conduct including strict client confidentiality. Provide client Intake services.
- Set up appointment schedule and help with transportation needs.
Qualifications You’ll Bring
- Knowledge of professional social work theories and concepts, including practical knowledge of professional social work practices. Ability to assess family and individual needs. Awareness of personal and professional boundaries, and ability to maintain them in a small community.
- Knowledge of community, tribal, county and state resources, and ability to maintain positive relations with those resources.
- Experience working with people affected by poverty and awareness of the issues that often go hand and hand with poverty.
- Effective oral and written communication skills.
- Ability to respond calmly to crisis and chaos. Resourceful, with strong problem-solving skills. Experience and knowledge in providing crisis intervention.
- Ability to maintain strict client confidentiality.
- Possession of personal and professional self-care skills.
What to Expect
- Extensive contact, communication and direct service with Tribal families.
- Requires coordination and communication with other Tribal, state and federal programs for the purpose of assessing services for families and coordinating services through case management.
- May require assertive communication on behalf of families.
- Requires creative problem solving under stressful or crisis conditions.
- Wide diversity of work situations includes working in numerous environments including, office, and clients' homes.
- Requires travel and transportation of clients.
