Job Summary:
This role plays a critical part in facilitating communication, coordinating services, and ensuring seamless integration of behavioral health care into a patient's overall treatment plan.
Duties and Responsibilities:
- Serve as a primary point of contact for patients, families, and interdisciplinary care teams.
- Coordinate behavioral health services with medical providers, therapists, and external agencies.
- Schedule assessments, therapy sessions, and follow-up appointments while ensuring timely communication of any changes.
Requirements:
- Bachelor's degree in psychology, social work, healthcare administration, or a related field.
- Prior experience in behavioral health, care coordination, or healthcare administration is highly desirable.
