Position Title: QA/QI Coordinator
Reports To: Chief Operating Officer
Purpose: To provide quality assurance and quality improvement direction and implementation to assure regulatory compliance to all Local and Federal Agencies rules and regulations.
POSITION RESPONSIBLITIES:
Ensure compliance with all policies and procedures, as well as local, state, and federal regulations.
- Conduct satisfaction and service reviews with consumers that are currently receiving services on a weekly, monthly, and quarterly basis
- Review and complete investigations weekly, monthly, and quarterly to ensure compliance
- Report all allegations of neglect, service inefficiencies and/or consumer dissatisfaction to the Director of Operations and Clinical Director immediately following notification
- Monitor unusual occurrences, report follow- up procedures, and reports monthly.
- Coordinate the development, implementation, and evaluate overall quality service delivery throughout all programs
- Conduct fact-finding investigations, including but not limited too gathering all relevant support documentation, completing and submitting reports
- Conduct detailed interviews of all witnesses with relevant information regarding the above- mentioned issues of concern
- Review service notes in an effort to maintain the standard of excellence within the medical record and service delivery of the agency and performed other assigned duties as necessary with in the realm of the Medical records department
- Manage complaints and suggestions
- Identify trends, review and analysis data to prioritize and recommend quality improvements and assurance.
MINIMUM QUALIFICATIONS:
- Master’s Degree in Social Work, Psychology or other clinical based discipline preferred
- Minimum of one year post-graduate experience is preferred
- Bachelor’s Degree from an accredited institution with at least two years experience working in the field of mental health required
- Must be able to pass a satisfactory background check, drug test, and child abuse registry
- Ability to successfully complete service-specific training courses
- Proficient in clinical theories and their practical application
- Computer literate; experience with MS Word and Excel
PREFERRED QUALITIES:
- Successful applicant will be well organized, have excellent time management skills and be able to work well both independently and with a team
- Have a valid drivers’ license, reliable transportation and insurance
