Job Title: Case Manager
Schedule: 8:00 am - 5:00 pm
The Teen Project, Inc., Carol's Sanctuary is a nonprofit serving at-risk, addicted, homeless, and human-trafficked women between the ages of 18, up until approximately 30 years old. Through the support of the County of Los Angeles, Department of Public Health, Substance Abuse Prevention and Control (SAPC), gracious donors, and other applicable funding sources, The Teen Project offers a unique innovative center that combines (if applicable): drug treatment, mental health, housing, workforce development, and education to springboard those lacking a family into a life of success.
Job Summary: A Case Manager ensures a collaborative and coordinated approach tailored to facilitate the continuity of care and promotes client progress towards self-management and autonomy. A Case Manager successfully links clients to services and resources that address individualized needs to attain treatment goals. A Case Manager plays a vital component in transitioning clients through the SUD system of care by complementing clinical services and promoting reintegration into the community.
Job Type: Full-time
Pay: $27.00 - $28.00 per hour
Expected hours: 40 per week
