Job Summary
Peachtree Hills Place, a premier senior living community, is seeking an experienced MDS Coordinator (RN) to oversee the assessment and care planning process for our skilled nursing residents. This role ensures compliance with federal and state regulations while optimizing reimbursement through accurate and timely Minimum Data Set (MDS) submissions.
Key Responsibilities:
- MDS Assessments & Documentation
- Coordinate and complete all MDS 3.0 assessments in accordance with CMS guidelines.
- Ensure accuracy in coding for resident assessments and care plans.
- Conduct resident interviews and collaborate with interdisciplinary teams.
- Care Planning & Compliance
- Develop and oversee individualized care plans based on assessment findings.
- Ensure compliance with state and federal regulations (CMS, OBRA, PDPM).
- Monitor quality measures and implement corrective actions as needed.
- Reimbursement & PDPM Management
- Optimize PatientDriven Payment Model (PDPM) reimbursement through accurate assessment and documentation.
- Work closely with the billing department to ensure timely submission of reimbursement claims.
- Collaboration & Leadership
- Educate staff on MDS processes and regulatory changes.
- Serve as a liaison between nursing, therapy, and administrative teams to ensure holistic resident care.
- Participate in Quality Assurance & Performance Improvement (QAPI) initiatives.
- Active RN or LPN license in the state of Georgia.
- Minimum 2 years of MDS experience in a skilled nursing facility.
- Strong knowledge of MDS 3.0, PDPM, OBRA, and Medicare guidelines.
- Proficiency in MDS software (PointClickCare or similar).
- Exceptional organizational and leadership skills.
