Job Title: Social Worker (LMSW, or LCSW)

Reports to: Program and Community Engagement Manager
Job Summary: The Transition Academy (TTA) is Jackson County, Missouri’s most comprehensive employment training program for high school special education students with disabilities. TTA is based in Kansas City, Missouri’s urban core and serves high school students with autism or developmental disabilities enrolled in Kansas City Public Schools (KCPS) and charter schools. TTA’s mission is to make economic inclusion a reality for youth with disabilities, especially for young people of color who have historically been failed by systems.
TTA is looking for a Social Worker to join our team, which includes an Occupational Therapist and a Licensed Professional Counselor. The Social Worker will make external referrals to organizations that will provide services and support for youth and families.
The Social Worker will partner with the Real World Learning Team and assist in classrooms to facilitate assessments as well as lead career exploration activities, such as interactive field trips and job skills activities.
Essential functions of the job may include but are not limited to the following:
- Highly skilled at developing case management procedures and processes that support programming and educational materials tailored for students with developmental disabilities
- Provides direct and indirect services to students with IEPs in a classroom setting
- Provide personal college and career guidance for students with IEPs, such as short-term individual, family or group social emotional/mental health counseling to prepare students for the workforce or college.
- Provide community resources and make referrals as needed to students and parents/guardians about services available within the community. Makes referrals to community providers, as appropriate.
- Maintains an accurate record of student, parent/guardian, and other contacts.
- Ability to travel between and among school sites throughout the city as necessary.
- Work collaboratively with multidisciplinary teams within each school building.
- Participate in team meetings, case conferences, and training sessions.
- Perform other duties as assigned by the supervisor.
EDUCATION OR EXPERIENCE:
Required: Bachelor’s degree in Social Work or related field (i.e.Sociology, Psychology, Counseling)
Preferred: Education/Certification: Master’s degree in Social Work, with an active license in LMSW or LCSW.
Special Knowledge/Skills: Psychosocial Assessments and treatment planning, human development, mental health, behavior interventions, relationship building, strong communication skills both orally and written, and working with students with disabilities in education settings, particularly within communities of color.
Salary:
$50,000-$65,000 per year
