Join the University of Chicago Medicine as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for Transitional Care Management (TCM) Virtual Visits Team (on-site) within our Care Transitions Clinic.
Eligible for a Sign-On Bonus up to $10,000Eligible for Relocation Assistance The Advanced Practice Provider will be responsible for comprehensive direct patient care management and will manage a caseload of patients by exercising autonomous decision making in the assessment, diagnosis, and initiation of delegated medical treatments. The APP will provide healthcare and patient services that may include educational, research, and clinical support, as well as complete administrative activities under the direction and supervision of the Director/leadership. The APP has acquired the knowledge, skills, and judgment necessary to assist the physician through organized instruction and supervised practice.The Transition of Care Management (TCM) APP will perform virtual patient care visits (to be done on-site), providing follow up care for adult patients post-hospital discharge. The goal of these visits is to ensure smooth transition back to Primary Care and Specialty providers, helping to minimize ED utilization and re-hospitalization when possible. This position will be housed in our Care Transitions Clinic where these TCM visits are conducted alongside other programs and services aimed at reducing acute care utilization. As such, there may be an opportunity to cross-train with other Care Transitions Clinic programs and services for in-person visits including in-person TCM visits along with other visit types.
Advanced Practice Provider
Essential Job Functions• Taking patient histories, conducting virtual visits, diagnosing illness• Patient evaluation and ordering and interpreting laboratory tests• Ability to exercise autonomy in medical decision-making, provide a broad range of diagnostic and therapeutic services
Nurse Practitioner Education and Experience Requirements:• Master’s of Science in Nursing• Current and/or eligibility for State of Illinois RN and APN licensure/registration • Must be certified as a Nurse Practitioner by the ANCC or AANP• Must have or be able to qualify for Illinois and DEA prescribing numbers• At least 1 year of APN experience preferred
Physician Assistant Education and Experience Requirements:• Completion of an accredited (physician assistant) educational program • Certification in ACLS and by the National Commission on the Certification of Physician Assistants (NCCPA) and current and/or eligibility for State of Illinois licensure/registration• At least 1 year of experience preferred
