About the Position
We are seeking a qualified Adult Home Health Nurse Specialist to join our team providing adult home care services in the Burlington, Winooski and South Burlington area.
As an Adult Home Health Nurse Specialist, you will be responsible for delivering basic nursing care to patients in their homes, under minimal clinical supervision. You will work collaboratively with our team of RNs, Physical Therapists, LPNs, LNAs, SLPs, and Social Workers to provide comprehensive care and support to patients.
- Key Responsibilities:
- Deliver basic nursing care to patients in their homes
- Collaborate with multidisciplinary team to develop and implement patient care plans
- Provide education and support to patients and their families
Qualifications
To be considered for this position, you must meet the following qualifications:
- A minimum of two years of experience in home/public health or hospice care
- Current LPN licensure or compact licensure recognized by the State of Vermont
- Excellent communication and interpersonal skills
