Collaborative Care Delivery
At The University of Vermont Health Network, we believe in delivering high-quality care through collaborative efforts among healthcare professionals. As a Home Care Nurse, you will work closely with a multidisciplinary team, including RNs, Physical Therapists, LPNs, LNAs, Occupational Therapists, Speech Language Pathologists, and Social Workers, to provide comprehensive care to patients in their homes.
Key Responsibilities
Deliver skilled nursing care to patients in their homes, working to the top of your licensure and utilizing evidence-based practices to improve patient outcomes. Our Per Diem positions are geared towards experienced clinicians who possess prior home health skilled nursing experience.
Qualifications and Benefits
Greater than two years of home or public health nursing experience is preferred, but a combination of education and experience that equips you with comparable knowledge and skills is also acceptable. Previous home care experience is desired but not required; we are willing to train and develop a candidate who demonstrates a willingness and desire to learn home health nursing. Our coverage area serves Chittenden and Grand Isle Counties and is segmented into four territories. Your assigned territory will be determined after the orientation period. We offer a competitive compensation package, including a $10,000 Sign On Bonus for full-time external applicants (prorated for part-time roles). Additionally, you will receive mileage reimbursement at Federal Gas Rates.
