JOB DESCRIPTION:
Under the direct supervision of the Care Management/Life Enrichment Director the Care Management Lead is responsible for planning and implementing quality of life (life enrichment) activities programs, both individual and group, that meet the needs of all Woodridge residents, seeking to resolve problems and conflicts and is responsible for a variety of social work services such as, but not limited to, performing tasks to support residents’ admission, stay, and discharge processes. Is able to seek out and initiate tasks that present themselves on a daily basis. Confidentiality is a must at all times.
EDUCATION:
Associates degree or equivalent education and work experience required. Ability to operate general office equipment. Must possess an overall knowledge and understanding of the aging process. Must also possess knowledge and computer experience in Microsoft Word and Excel.
EXPERIENCE:
Minimum of one years’ experience in a related field or a nursing home/health care facility.
