Family Services Manager - LICSW - Home Health and Hospice

POSITION SUMMARY
The Family Services Manager is the direct supervisor for team leaders of non-clinical departments within the Hospice Interdisciplinary Team including Social Work, Spiritual Care, Volunteer Coordination and Bereavement. The Interdisciplinary Services Manager collaborates with the Community Hospice and McClure Miller Respite House Clinical, LNA and Administrative Managers or delegates, as well as the Hospice Physician, under the supervision of the Hospice and Palliative Care Program Director to assure compliant and quality care provision that meets the social, spiritual and bereavement needs of Hospice clients and families. This position contributes to the success of agency and network goals related to palliative care through the generation of billable services, committee work and public engagements, and acts as a liaison between discipline leads with senior leaders in Home Health and Hospice.


REPORTS TO: DIRECTOR OF HOSPICE

MINIMUM QUALIFICATION STANDARDS
I. Education:
Masters’s Degree in Social Work, VT LICSW credentials required.

II. Experience:
Five years of relevant hospice experience, or a combination of education and experience from which comparable knowledge and skills are acquired. Two years of experience supervising or managing employees

III. Knowledge:
a. Knowledge of the business of hospice.
b. Understanding of the social, spiritual, and bereavement issues related to those with a life limiting condition as well as their families.
IV. License Requirements:
Certification in Hospice & Palliative Care or other related specialty preferred.
V. General
  • Must demonstrate and maintain any required competency
  • Able to speak, read and write in English
  • Be honest, self-directed, possess initiative, display good judgment and a positive attitude toward daily activities
  • Have the ability to communicate clearly and effectively both orally and in written work
  • Be able to work cooperatively and in a productive manner with organization’s employees, clients, customers and the general public
  • Maintain satisfactory standing with the Adult Abuse Registry, Child Abuse and Neglect Registry, Vermont Criminal Information Center and Office of Inspector General
  • Be free from the illegal use and effects of drugs and alcohol in the workplace
  • Be able to carry out the functions of this job without risk of harm to the health and safety of self and others
  • Hold a valid driver’s license and have a reliable, registered vehicle with insurance as required by Organization, and appropriate to position
  • Excellent working knowledge of pain and symptom management
  • Demonstrated commitment to an Interdisciplinary model of care
  • Demonstrated interviewing, counseling and basic teaching skills
  • Ability to explain medical problems and treatments in accurate and understandable language
  • Ability to deal effectively with a wide range of individuals both in and outside of the organization, including physicians, other community health care and social service organizations, and various Home Health & Hospice personnel, requiring good coordinating/liaison, consulting and patient advocacy skills
  • Administrative and clinical supervision is received from the Director of Hospice Services
WORKING CONDITIONS:
1. Traveling may be required
2. May work in community based settings, and clients’ homes and outdoors
3. May work during non-business hours
4. May experience:
  • Hostile and emotionally upset clients, family members, visitors and staff
  • Stress due to a demanding profession
  • An exposure to blood-borne pathogens
  • A variety of environmental exposures such as inclement weather, animals, firearms, tobacco use, dust, and other client based activities and preferences

ESSENTIAL PHYSICAL REQUIREMENTS:
1. Must walk, sit, stand, bend, lift and move intermittently during working hours
2. Must have the ability to push, pull and lift from the floor, up to a total of 50 pounds and be able to move with the weight, as to organization’s standards
3. Climbing-be able to navigate stairs of varying heights and degrees
4. Balancing-be able to maintain balance
5. Kneeling, crouching, stooping, twisting, reaching below waist level, reaching overhead and reaching torso level
6. Manual finger dexterity-will need dexterity to perform duties and use equipment
7. Visual acuity-will need sufficient eyesight to drive a car, use office equipment and perform duties
8. Auditory acuity- will need to assist individuals in person and by telephone

PERFORMANCE REQUIREMENTS:

Essential Functions and Responsibilities
  • Fosters proficient to expert care delivery consistent with Values Competency of the National Hospice and Palliative Care Organization (NHPCO) and Medicare Conditions of Participation.
  • Support, develop and supervise an interdisciplinary team consisting of team leaders in social work, volunteer coordination, bereavement, and spiritual care
  • Recruits, trains, hires, evaluates and coaches team members
  • Ensures compliance with Federal Conditions of Participation for Hospice
  • Directly and indirectly ensures compliance with visit frequency, services delivered, and completion of clinical documentation
  • Supports continued growth and excellence of clinical skills and staff development
  • Responsible, in partnership with the hospice Clinical Managers for Interdisciplinary group (IDG) process – discharges, admissions, re-certifications, reviews
  • Responsible for the assignment of care, clinical integrity of practice, and flow of documentation
  • Coordinate with designated departments and IDG team members new staff orientation/training, staff development programs and team building programs
  • Monitor operations efficiency, create successful productivity standards, and support staff to enhance productivity capacity, efficiency and effectiveness of care delivery
  • Participate in ongoing performance improvement including the design and revision of policies, structures and processes
  • Responsible for identifying and implementing Quality Improvement Projects
  • Participates in chart reviews and auditing as part of Education/Quality Team members
  • Participates in staff, volunteer, community training and educational programs
  • Develops and/or participates in education programs and workshops for clients, staff and community at large.
  • Demonstrates respectful and effective communication in all interactions
  • Complies with organization policies to protect confidentiality, integrity, availability and security of all protected health information. Report breach of security policy to Organization’s Security Official
  • Conducts themselves in a safe manner by adhering to all safety practices, rules and standards
  • Demonstrates a commitment to quality and proactively seeks improvements to systems and processes
  • Reports to work on time, provides advance notice for time off, accurately reports time and attendance and appropriately manages paid time off (if applicable) in accordance with organization policy
  • Performs duties as assigned that may be outside their principle responsibilities
  • Effectively communicates to transfer information, provide guidance and resolve conflicts
  • Reports concerns and complaints regarding abuse, neglect, and exploitation in accordance with Vermont State Law and Organization
  • Enhance or improve organization services by participating in team meetings, projects and committees
  • Contributes to a collaborative environment and responds in a constructive manner to change
  • Documents the provision care and services in accordance to Organization policy
  • Upholds all expected behaviors in alignment with the University of Vermont Health Network Code of Conduct
