About the Role
We are seeking a compassionate and dedicated LPN to join our growing Adult Home Health team at UVM Home Health & Hospice. In this role, you'll play a vital part in helping patients receive high-quality care in the comfort of their own homes.
Your Key Responsibilities:
- Provide skilled nursing care under the supervision of an RN, following established plans of care.
- Assess patients' physical, emotional, and social needs.
- Administer medications, wound care, and other treatments as prescribed.
- Educate patients and families on healthcare needs and self-management strategies to promote independence and well-being.
- Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
About You:
- You hold an active LPN license in Vermont.
- You have a minimum of two years of directly related home/public health, hospice and/or maternal/child health nursing experience.
- You possess strong assessment, communication, and interpersonal skills, with a patient-centered approach.
- You excel in teamwork and collaboration, working effectively with colleagues to achieve shared goals.
