Our Population Health Services Organization (PHSO) at UVM Health Network is excited to announce the expansion of our Integrated Care Management services in the Burlington, VT area! We are seeking candidates with Social Work or RN experience to lead this newly expanded team.
JOB DESCRIPTION:
Manager of PHSO Care Management leads interdisciplinary core teams in delivering comprehensive, consistent, timely, effective, and patient/family centric care. The Manager is responsible for the oversight of the staff and daily operations for the PHSO Care Management department. Provides training, support to direct reports and encourages individual and team development. Participates in process improvement: ensuring policies, procedures, and customer-service are constantly monitored, evaluated, and supported. Assist the Director in ensuring regulatory compliance. As part of the management team, the manager assists the director in fulfilling departmental goals.
EDUCATION:
Master's in social work from an accredited school of social work.
LICSW preferred.
OR
Current RN licensure or compact licensure recognized by the State of Vermont is required.
The University of Vermont Health Network is moving toward an all Bachelor of Science in Nursing (BSN) workforce. Current internal RNs are strongly encouraged to pursue a BSN to support this initiative.
Case management accreditation by a nationally recognized accrediting body for case management (examples: CCM, ACM or ANCC certifications) preferred.
EXPERIENCE:
3-5 years of leadership experience in a healthcare setting required
3-5 years of clinical experience in a healthcare setting (or equivalent) required
Care Management/Care Coordination experience required
