The Population Health Services Organization (PHSO) is hiring for a Resource Coordinator to join their team! This position will support Adult Primary Care in Barre, VT.
Please note: This work environment will be hybrid, allowing for remote work and requiring a weekly onsite presence in Barre, VT.
Job Description
Coordinates the care and services of selected patient populations across the continuum of illness, with a focus on addressing the Social Determinants of Health; promotes effective utilization and monitoring of health care resources; and assumes a role with the interdisciplinary teams to achieve optimal patient-centered, clinical and resource outcomes. Works within care teams to address barriers to treatment, health, wellness, prevention, improvement and outcomes. Serves as an important link between the patient, the healthcare teams, the payors and the community.
EDUCATION
Associates Degree in Human Services. Bachelor's level degree in Human Services preferred. Candidates with high school diploma and relevant work experience in a healthcare setting may be considered.
Experience
2-3 years of experience in a healthcare setting (or equivalent)
Care Coordination Experience Highly Preferred. Customer Service Experience Preferred
