Ou Population Health Services Organization Quality Team with The University of Vermont Health Network is hiring a Resource Coordinator to join their team!

As a PHSO Resource Coordinator, you will play a pivotal role in driving improvement in the health of our primary care population. Your responsibilities will include:
Utilizing data and reporting to identify patient care needs.
Communicating with patients and care teams through a variety of ways including printed letters, phone calls, and electronic health record messages.
Proactively reaching out to patients to discuss and schedule appropriate visits or screenings.
Receiving incoming patient calls with the ability to navigate the electronic health record to assess reason for call, specific care needs and appropriate actions.
Acting as an extension of the patient care teams, creating a seamless communication experience for the patient.
Maintaining accurate and up to date electronic health records including documentation of key services and screenings.
Please note: Weekly onsite presence will be required at the IDX Building in South Burlington, VT with flexibility for working remotely at times.
JOB SUMMARY
Coordinates the care and services of selected patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources; and assumes a role with the interdisciplinary teams to achieve optimal patient-centered, clinical and resource outcomes. Works within care teams to address barriers to treatment, health, wellness, prevention, improvement and outcomes. Serves as an important link between the patient, the healthcare teams, the payers and the community.
EDUCATION
Associates Degree in Human Services or 2-3 years of experience in a healthcare setting (or equivalent).Bachelor’s level degree in Human Services preferred.
EXPERIENCE
Care coordination or clinic office experience preferred. Customer service preferred.
SKILLS
Strong communication skills are essential. We are looking for a reliable candidate who can navigate patient conversations seamlessly.
