Board Certified Family Physician with Geriatric Focus
Clinical Scholar Path, Assistant/Associate/Professor
The Department of Family Medicine at the University of Vermont Larner College of Medicine and The University of Vermont Medical Center is seeking a board certified/eligible family physician with additional training in Geriatrics (Fellowship Training or Advanced Certification) to join the full-time faculty. This appointment at the University of Vermont will be at the Assistant/Associate/Professor level (1.0 FTE) in the Clinical Scholar Pathway commensurate with experience. The selected candidate will join a busy and collegial program. The position requires clinical focus within well-established medical homes. These Patient-Centered Medical Homes (PCMHs) have also been through a workforce transformation and are practicing and teaching population health management. Experience in population health, advanced care delivery, Care Teams and teaching is encouraged. We seek a rich, diverse workforce and encourage everyone to explore our department and state.
Priority will be given to applicants who have experience providing family medicine clinical services in an academic setting. Our faculty participates with teaching in our family medicine residency education program and our medical student educational programs. Our 6:6:6 residency program is fully accredited, and is growing (expanding to a 7/7/7 program over the next 3 years). Our program supports academic and professional growth while affording excellent quality of life.
This position will reside primarily in the Family Medicine- Milton Primary Care Medical Home, which is our Family Medicine Residency Clinic Site, with 45% Primary Care Medical Home Effort (traditional full-scope Family Medicine practice). The physician will work within a population-health based model of care with their own risk-adjusted panel of patients in a high-value care, team-based setting. In addition, this physician will work 2 days per week (40% effort) in the Nursing Home setting, based at the Green Mountain Nursing Home (GMNH), a 73-bed Nursing Home facility located in beautiful Colchester, Vermont. This position will provide long-term care and subacute care to the population of patients at GMNH with a team of Family Medicine Providers under the direction of our Family Medicine Medical Director at GMNH and the Health Network Division Chief of Geriatrics. The position will also have 12% protected academic time to lead the geriatrics curriculum for the UVM Family Medicine Residency Program, including clinical teaching at the Nursing Home with resident physicians and coordination of the Family Medicine Residency Geriatrics rotation. The geriatric rotation is robust required rotation with community connections including liaison work with AgeWell, Meals on Wheels, UVM HN Home Health and Hospice, and other community-based organizations. Finally, this position will lead twice monthly teaching home visits (Two 4-hour Home Visit sessions per month) with an established home visit practice based out of Family Medicine- Milton with an additional protected 3% effort supporting Home Visit teaching. The total effort is a full time, 1.0 position with a wonderful balance of primary care and geriatrics teaching and clinical care.
