What are the responsibilities and job description for the PHSO Resource Coordinator position at The University of Vermont Medical Center?
Our Population Health Services Organization (PHSO) at UVM Health Network is excited to announce the expansion of our Integrated Care Management services in the Burlington, VT area! We are seeking candidates with a background in Human Services to join our team as a Resource Coordinator.
Please note: This work environment will by hybrid, allowing for remote work and requiring an onsite presence in the Burlington, VT area weekly.
Job Summary
Coordinates the care and services of selected patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources; and assumes a role with the interdisciplinary teams to achieve optimal patient-centered, clinical and resource outcomes. Works within care teams to address barriers to treatment, health, wellness, prevention, improvement and outcomes. Serves as an important link between the patient, the healthcare teams, the payers and the community.
Education
Associates Degree in Human Services or 2-3 years of experience in a healthcare setting (or equivalent). Bachelor’s level degree in Human Services preferred.
Experience
Care Coordination experience preferred.
Customer Service Experience Preferred.
