Job Summary:
The SSVF Healthcare Navigator assists Veterans in accessing VA healthcare benefits or alternative community healthcare services if they are not eligible for VA care. The Navigator provides case management, care coordination, health education, interdisciplinary collaboration, and administrative duties. This role works closely with Veterans, primary care providers, and interdisciplinary teams to ensure timely, comprehensive, and equitable care.
Essential Functions:
Connect Veterans to VA healthcare benefits or appropriate community healthcare services
Conduct assessments in collaboration with interdisciplinary teams, Veterans, family members, and significant others.
Provide holistic case management services, including intake assessment, benefits evaluation, goal setting, long-term case planning, tenant education, advocacy, and referrals.
Coordinate with VA, Department of Labor (DOL), and other government/community agencies as needed.
Regularly review and adjust Veterans’ care plans.
Conduct non-clinical barrier assessments.
Maintain accurate and up-to-date client records.
Participate in team meetings, case conferences, and training sessions.
Qualifications
- Strong understanding of patient service and discharge planning
- Proficient in medical terminology and maintaining medical records
- Experience with case management and acute care settings
- Knowledge of HIPAA regulations and compliance
- Familiarity with intake processes and care plan development
- Preferred experience in hospital environments and social work, particularly in behavioral health and pediatrics
Qualifications:
- Must be proficient with computers, computer applications, and case management software
- Strong written and verbal communication skills
- Ability to work both independently and as a part of a team.
- Excellent organizational, problem-solving, and decision-making skills.
- Knowledge of confidentiality laws and healthcare regulations.
- At least two years of experience in healthcare or social services is preferred.
- Bachelor's/Master’s degree in Social Work or a related field, or at least 2 years experience in a related field preferred.
- Must possess organizational, planning, information gathering, information monitoring, problem analysis, and problem-solving skills.
- Must possess judgment and decision-making ability, initiative, confidentiality, and attention to detail and accuracy
- Possesses excellent judgment
- Valid New York State driver’s license is required.
Job Types: Full-time, Part-time
Pay: $23.00 - $26.50 per hour
