Are you a detail-oriented nursing professional with a passion for long-term care and quality resident outcomes? We’re looking for a dedicated Minimum Data Set (MDS) Nurse to join our team! This key role is responsible for the coordination, completion, and accuracy of MDS assessments, resident care plans, and medical documentation in accordance with Medicare and state guidelines.
🔍 Key Responsibilities:
- Coordinate and complete MDS assessments and care plans using our computerized system
- Ensure timely and accurate MDS completion following HCFA and Medicare regulations
- Participate in resident and family care plan conferences
- Maintain effective communication across departments regarding medical record documentation
- Monitor and review charts for completeness, updating care plans as needed
- Prepare and submit required QAPI reports
- Assist with the development of medical records policies and procedures
- Respond to emergency calls and support other healthcare staff as needed
- Maintain professional appearance and uphold high standards of resident care
✅ Requirements:

Active RN or LPN license in the State of Tennessee (Required)
- Minimum 2 years of MDS experience (Required)
- Experience in a long-term care setting (Preferred)
- Familiarity with ICD-10 coding and electronic health record systems such as AOD
- Strong knowledge of geriatric care and resident rights
- Excellent communication and interpersonal skills
