Major Duties and Responsibilities
- Supervise all resident care, services and unit activities (routinely check on residents, assess the daily needs of the residents, execute necessary facility policies and procedures regarding admission, transfers, discharges, critically ill and deaths).
- Coordinate resident care maintaining and implementing rehabilitative, restorative, and supportive programs.
- Maintain accurate, current resident charts via computer including physician visits, orders, and stop orders, resident care plans, medication and treatment records, appointment scheduling and calendar assignment.
- Provide feedback to the Unit Manager regarding resident care and status, report all concerns or complaints to Unit Manger/supervisor immediately.
- Receive Charge Nurse report from previous shift and create a report for the following shift (including a narcotic count and transfer of keys).
- Execute all appropriate nursing department policies and procedures in the delivery of care.
- Accompany physicians on resident rounds, reviewing the chart for documentation and signature as needed.
- Monitor floors for proper turning/positioning, bathing, call lights, cleanliness of rooms/ halls, monitoring of shower rooms, med carts, etc.
- Administer medication, tube feeding and IV’s (according to license and training status) and record according to physician’s orders, facility policy and safe nursing practice.
- Execute treatments including dressing changes and O2 respiratory therapy and records according to physician’s orders, facility policy and safe nursing practice.
- Monitor and respond to residents’ nutritional needs, weight gain and loss on an ongoing basis, assuring that assistance is provided to meet nutritional and hydration needs.
- Conduct nursing review on admission, return from hospital leave of absence on change in condition and as appropriate.
- Assist with admission, transfer and discharging of residents as required.
- Ensure that resident rooms are ready for new admissions, notify floor nurse of admissions upon receipt of email.
- Consult with the UM or DON about the possible use of restraints when necessary.
- Complete incident/accident reports as necessary, ensure statements accompany incident reports.
- Communicate daily, by fax and telephone, with pharmacy about any changes in physician orders.
- Communicate daily with physicians regarding new admissions, resident status changes, new resident symptoms, problems, lab, x-ray reports, etc.
- Notify the appropriate family member(s), responsible party or sponsor when there is a change in the resident’s condition and contact funeral homes when requested by the family of the resident.
- Coordinate scheduling of resident appointments and transportation, ensure placement on appointment calendar.
- Communicate with residents and sponsors regarding resident condition, orders, treatment, appointments, and plan of care.
- Maintain confidentiality of all resident care information.
- Assure that all residents receive care in a manner and in an environment that maintains or enhances their quality of life.
- Ensure that all resident care rooms, treatment areas, etc. are maintained in a clean, safe, and sanitary manner.
- Maintain a written record of the resident’s complaint/grievance and direct these to the UM or DON for review of action(s) taken to resolve the complaint/grievance.
- Ensure compliance by Nursing Assistants with all administrative and personnel policies and procedures
- Ensure that a sufficient number of nursing assistants are available for the shift to ensure that routine nursing care is provided to meet the daily nursing care needs of each resident.
- Instruct and oversee Nursing Assistants in delivery of new and/or changes in resident care orders (rehabilitative, restorative, or supportive services, medical orders and plan of care).
- Plan, organize, and direct the activities of Nursing Assistants, including scheduling, attendance, and assignment of duties and responsibilities.
- Implement and maintain the orientation program for new employees on duty.
- Assist in evaluating the performance reviews of Nursing Assistants.
- Give nursing report to Nursing Assistants about the individual status of their assigned residents.
- Ensure that disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, veteran status, handicap, or marital status in accordance with company policies.
- Ensure that all nursing personnel are in compliance with residents’ rights.
- Ensure that nursing service personnel follow established infection control (handwashing) procedures and isolation precautions when necessary.
- Assure that the stock levels of medications, medical supplies, equipment, etc. are maintained on the premises at all times adequately to meet the needs of the residents.
- Recommend to the UM the equipment and supply needs of the residents.
- Participate in Continuous Quality Improvement/Performance Improvement Programs as required.
- Report/document any accidents or incidents assisting in the investigation of these occurrences.
- Maintain effective communication with residents, sponsors, department personnel, other services and administration.
- Create and maintain an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment throughout the facility.
- Maintain a good rapport and act as liaison with families and residents.
- Develop and maintain a good working rapport with inter-department personnel to assure that nursing services and activities can be adequately maintained to meet resident needs.
- Attend and participate in continuing education programs designed to keep you abreast of changes in profession as well as to maintain current licensure.
- Attend required in-services regarding various topics and Corporate Compliance.
