Key Responsibilities
- Focus on engaging clients from the time of the referral through the entire course of the clients treatment services, including providing periodic check-ins and Recovery Management Check-ups, as appropriate.
- Participate in treatment planning activities, as appropriate for each client.
- Assist clients in seeking public or private insurance, whenever appropriate.
- Discuss clients health-related social needs and offer resource and referral services specifically tailored to that client.
- Meet with clients and their families/significant others to provide education, support and role modeling regarding recovery.
- Provide linkage and navigation assistance for clients and their families/significant others so they can access all relevant community services.
- Help clients and their families identify and access other Village clinical services and/or other community resources. Screening will provide the basis for referrals, and navigation services will include advocacy and follow-up. Other Village services may include specialty treatment for children and adults.
- Meet individually and in groups with clients to assist in forming linkages with other community resources.
- Take steps to ensure client engagement from referral through transfer/discharge from service, including initial outreach to clients upon initial participation in MAT program.
- Participate in educational programming and individual services to clients and families to provide information about recovery, addiction, use of Naloxone, and other relevant topics.
- Serve as a resource for case management services to clients and their families.
- Assist clients in connecting with and gaining eligibility for entitlement programs, including public and/or private insurance programs.
- Develop and maintain collaborative relationships with other community agencies, organizations and referral sources in consultation with supervisor to provide comprehensive and effective treatment services for clients (i.e. schools, courts, hospitals, etc.)
- Maintain effective, positive working relationships with referral sources, including DMHAS, DOC and CSSD.
- Meet all standards for timeliness and accuracy in performing data and reporting functions, including communication with referral sources.
- Produce and communicate timely and accurate progress reports, court reports and other documents, as appropriate.
EDUCATION
- Successful completion of the Connecticut Community for Addiction Recoverys (CCAR) Recovery Coach Academy or equivalent.
- 2 years experience as a Recovery Coach.
- Ability to communicate comfortably with clients, family members and significant others, and community resources and professionals about recovery-oriented topics, including the RCs personal recovery.
- Organized and able to work independently with minimal supervision.
- Flexible and able to multi-task.
- Capable to function capably with relevant computer software, including word processing electronic health records. Ability to learn and work with specific applications used by The Village and its referral sources is necessary.
- Proven literacy and communication skills.
- Must be culturally competent to serve diverse client population; bi-lingual (English/Spanish) strongly preferred.
