**Job Summary**
The Village Health & Rehabilitation is seeking an experienced Registered Nurse (RN) to lead our Sub-Acute Unit. As a Sub-Acute Unit Leader, you will be responsible for providing high-quality patient care, supervising a team of nursing staff, and collaborating with interdisciplinary teams to develop and implement individualized care plans.
About the Role
- Provide patient-centered care to residents in accordance with established policies and procedures.
- Supervise and evaluate the performance of nursing staff, identifying areas for improvement and implementing corrective action as needed.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Ensure that our facility meets the highest standards of quality and safety.
Responsibilities
- Conduct daily nursing rounds to evaluate resident care and identify areas for improvement.
- Review and revise resident care plans to ensure they meet the individual needs of each resident.
- Collaborate with healthcare providers to develop and implement individualized care plans.
- Supervise and evaluate the performance of nursing staff, identifying areas for improvement and implementing corrective action as needed.
Requirements
- Demonstrated experience in a long-term care or sub-acute setting.
- Strong leadership and communication skills.
- Ability to work effectively in a fast-paced environment.
- Current licensure as a Registered Nurse in the state of practice.
