This Opportunity: The Village Life Center is seeking individuals who can provide Targeted Case Management & Mental Health Rehabilitative Services including Skills Building and Intervention Services.
Note: We are seeking candidates who can serve clients south of San Antonio (such as Uvalde, LaPryor, Hondo, etc) OR north of San Antonio (such as New Braunfels, Belverde)
Minimum Qualifications:
- Bachelor's or Master’s degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
QMHP Responsibilities:
As a QMHP your key responsibilities will include:
- Deliver in-home, in-school, in-community and virtual behavioral mental health services.
- Provide therapeutic interventions to enhance and foster client’s optimal functioning and interactions in home and community.
- Assist clients in restoring, rehabilitating and developing social and interpersonal skills.
- Identify, enhance, and establish support networks, community awareness, coping strategies, and improve functioning in the individual’s social environments (home, work, school).
- Help restore, rehabilitate, and support the development of daily living skills.
- Help improve each client’s self-management of the negative effects of psychiatric or emotional symptoms.
- Assists clients in identifying, effectively responding to or avoiding identified precursors (“triggers”) that lead to functional impairments.
- Complete CANS/ANSA and update treatment plans.
- Continually assess client needs, service availability and appropriateness.
- Communicate client’s status on a regular basis, including, but not limited to changes in mood or functioning, medical issues and all crisis and health and safety issues.
- Provide Crisis Intervention services to recipients as needed according to agency policies, procedures, and service definitions.
- Work cooperatively with treatment team members and the family in the provision of services to meet the goals developed in the Person-Centered Plan.
- Provide the appropriate documentation for service delivery including treatment goals, progress notes and submit said documentation as specified by agency standards.
- Maintain caseload and well-organized schedule to meet productivity expectations
- Attend weekly staff meetings for agency communication, clinical consultation and training.
- Participate in collaborative meetings with direct supervisor, other agencies or community and natural supports as needed.
- Be knowledgeable of and adept at producing effective, clinical documentation.
- Attend all office trainings as well as regular clinical supervision sessions with Clinical Manager.
- Document services provided regularly, concisely, and completely in accordance with clinical standards and State of Texas guidelines by office deadline. Must have insured vehicle, cell phone and access to a computer/laptop outside of office use).
Job Environment: The Qualified Mental Health Professional (CS) will generally perform responsibilities in the office, community, home and virtually. However, the majority of services provided will be in the client’s community and home environment.
