Job Summary
The Social Services Assistant plays a vital role in supporting individuals and families in navigating various social services programs. This position involves working closely with clients to assess their needs, provide guidance, and connect them with resources that promote their well-being. The ideal candidate will possess a strong background in behavioral health, social work, or related fields, demonstrating a commitment to improving the lives of those they serve.
Responsibilities
- Maintain accurate records of client interactions and progress towards goals.
- Participate in training and professional development opportunities to enhance skills relevant to social services.
- Regular and punctual attendance
- Report any, and all, Resident psychosocial changes or concerns immediately to Director of Social Services
- Perform timely documentation of Social Service interventions and assessments in PCC system
- Assist with coordinating smooth and safe Resident discharges into the community, Assisted Living, or the Retirement Center
- Assist with occurrences of theft/loss reports and provide any follow-up that may occur
- Assist with coordinating Resident room changes
- Participate in regularly scheduled Resident Care Plan Conferences, PPS meetings and other meetings as designated by Director of Social Services
- Notify new admissions of the time and date of care plan meetings by giving them written notice of the meeting at least 48 hours in advance
- At beginning of each month check eligibility and print report for those Resident’s who are on Medi-Cal; IEHP or Molina
- Assist with helping to cover Admissions department when the Admissions Director is out of the building (this would include but is not limited to: tours, inquiries, admission packets)
Qualifications
- A background in social work, behavioral health, or a related field is preferred.
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Ability to work independently as well as part of a team.
- Proficient in using technology for documentation and communication purposes.
- A commitment to maintaining confidentiality and ethical standards in all interactions.
- AA degree or 2 years working in psych/social environment-preferably with seniors
- Intermediate knowledge of Social Services functions
- Excellent verbal and written communication skills
- Goal and task oriented
- Ability to be self-directed, as well as, have good inter-personal skills
- Good problem solving and time management skills
